Requesting Medical Records
Medical records include a patient’s medical history, pathology, radiology, lab reports and operative reports of treatments, and medical services. Patients at Vascular Health Clinics can obtain copies by submitting a form to the Vascular Health Clinics Medical Records Release of Information Unit.
You may receive your medical records by mail or secure email. If you request electronic records, you will receive a link via email with access to a secure delivery server.
To request your patient medical record:
- Download and complete a medical information release authorization form
- Print and sign the completed form
- Fax the form to 989-607-1314, Attn: Vascular Health Clinics Release of Information
Faxing the form to us is the fastest and easiest way to obtain copies of your records. However, you may also request a copy of your records by sending a completed and signed printout of the medical information release authorization form to:

If the patient is a minor or unable to give consent, the signature of a parent, guardian, or other legal representative is required.
Additional authorization may also be required for the release of specifically protected or privileged information. Certain information can take up to 30 days for processing. Contact the Release of Information Unit at 989-254-6426 with questions about specific requests.
Vascular Health Clinics does not provide birth or death certificates. To request medical records of a deceased patient, the request must be accompanied by authorization from the executor of the estate.
